What we do

Structured decision records that people will actually write. Each decision captures the question, the options with their genuine trade-offs, the evidence and its quality, the assumptions the decision rests on, who decided and who disagreed, and the specific conditions that should trigger a revisit. It links decisions to each other so consequences are traceable, and it schedules the revisit automatically. Crucially it is fast to fill in, because the failure mode of every decision-log initiative is that writing it is tedious and it stops after six weeks.
